PA MAP-IT which stands for Manage, Assess, Plan, Intervene and Teach is a pilot project with the goal being: The prevention and treatment of healthcare acquired pressure ulcers across the continuum through the collaboration of acute and post - acute care providers.

  • MAP-IT for Healthy Skin website, a clearinghouse for information that pertains to the prevention and treatment of pressure ulcers
  • Development of two local continuums of care, each including one acute care facility, multiple skilled nursing homes, personal care homes and home health care/hospice agencies
